Trigger Point Injections (TPI)
Releasing the Physical Drivers of Pain
Headache disorders rarely exist in isolation. For many, the epicenter of a migraine is fueled by intensely tight knots of muscle, known as trigger points, in the neck, shoulders, and upper back. At Irvine Headache Care, we use Trigger Point Injections to deactivate these focal points of tension, providing both immediate relief and long-term structural improvement.
The Specialist Approach
While many general clinics offer trigger point therapy, Dr. Cheng applies a specialized neurological lens to every injection.
Precise Identification We do not simply massage the area. We palpate and identify the exact motor points that are referring pain upward into your head and jaw.
Breaking the Feedback Loop Muscle tension and migraine often create a cycle where the migraine causes the neck to tense, and the neck tension triggers more migraine. TPI interrupts this loop.
Medication Options We typically utilize a local anesthetic to reset the muscle fiber. Because we are a physician-only practice, your treatment is tailored to your specific sensitivity and clinical needs.
Conditions That Benefit
Cervicogenic Headaches:
Headaches originating from issues in the neck.
Myofascial Pain Syndrome
Chronic muscle pain and knots in the trapezius or levator scapulae.
Tension-Type Headaches
That vice-like grip around the head often driven by shoulder tension.
Migraine Prodrome
Addressing the neck stiffness that signals an oncoming attack.
What to expect?
During your physical exam, Dr. Cheng identifies specific knots through manual palpation. A very fine needle is then used to inject a small amount of anesthetic directly into the trigger point.
You may feel a brief muscle twitch or a referred sensation, which is a positive sign that the trigger point has been successfully reached and is releasing. Similar to our nerve blocks, you might experience a brief stinging or burning as the medication is administered, followed by a localized sense of relaxation in the muscle. There is no recovery time required, and many patients feel lighter and more mobile before leaving the office.
A Note on Navigating Insurance Obstacles
The traditional insurance model often creates barriers to consistent relief. Many carriers now frequently deny coverage for Trigger Point Injections or place arbitrary limits on how often you can receive them, regardless of your clinical needs.
At Irvine Headache Care, we have removed these obstacles. By choosing our direct-pay model, you are no longer restricted by insurance calendars or pre-authorization delays.
Freedom from Frequency Limits We do not wait for an insurance company’s permission to treat a flare-up. If your condition requires more frequent care to maintain your quality of life, we proceed based on your clinical needs.
Transparent Pricing We provide a clear, flat fee for every TPI session. This eliminates the risk of surprise bills or retrospective denials, allowing you to focus entirely on your recovery.
Out-of-Network Reimbursement While we do not contract with insurance, we provide a comprehensive Superbill for every procedure. You can submit this to your insurance provider for potential out-of-network reimbursement.
Frequently Asked Questions about Trigger Point Injections
-
Nerve blocks target the nerves to stop pain signaling, whereas Trigger Point Injections target the muscles to release physical tension. Often, we combine these two treatments to address both the neurological and muscular components of your headache.
-
Yes. In many cases, addressing muscle knots through TPI can complement the preventative work of Botox, especially if you have significant neck-driven triggers.
-
Insurance has become increasingly difficult regarding interventional pain management. We skip the hassle of pre-authorizations and provide a flat, transparent price for your TPI session. We provide a Superbill that you can submit to your PPO for potential out-of-network reimbursement.
-
Every session is customized to your physical exam. Dr. Cheng identifies the specific active trigger points in your neck and shoulders that are contributing to your headache. We then treat only those necessary points to achieve maximal relief while prioritizing your comfort.
-
Many patients experience an immediate release of tension and an improved range of motion before leaving the office. While the local anesthetic provides the initial "quieting" of the muscle, the long term goal is to break the chronic cycle of inflammation and tightness.
-
In most cases, you do not need to alter your medication routine. Unlike more invasive surgical procedures, TPI is a minimally invasive office treatment. If you are taking specific blood thinning medications, we will discuss any necessary precautions during your initial consultation.
